In a landmark achievement for South Africa’s public healthcare system, a team of doctors at Limpopo’s Mankweng Hospital has successfully separated conjoined twins in a highly complex operation—an accomplishment rarely seen outside major tertiary hospitals.
President Cyril Ramaphosa hailed the procedure as “miraculous,” praising lead surgeon Dr Nyaweleni Tshifularo and his multidisciplinary team for delivering world-class medical care in a rural setting.
Historic First for Rural Healthcare
The twins, born on 28 January 2026, underwent the intricate surgery at Mankweng Hospital, marking what officials describe as the first time such a high-level procedure has been successfully performed at a rural public hospital in South Africa.
“This is an unbelievable and complicated operation that would normally be done at top-class hospitals,” President Ramaphosa said, drawing parallels with the historic heart transplant performed by Dr Christiaan Barnard in the 1960s.
The successful outcome challenges long-standing perceptions about the limits of rural healthcare infrastructure.
A Breakthrough for Public Health Capacity
The operation is being seen as a powerful demonstration of the growing capability of South Africa’s public health system, particularly in underserved regions.
“I can just imagine how difficult the operation was. The whole nation is filled with pride that a rural public hospital has achieved this,” the President said.
The twins are now fully separated and in stable condition, recovering in the hospital’s neonatal intensive care unit under close medical supervision.

The procedure was led by Dr Tshifularo, supported by a highly coordinated multidisciplinary team of surgeons, anaesthetists, and specialised medical staff.

The success was also made possible through targeted support from the Limpopo provincial government.
President Ramaphosa commended Premier Dr Phophi Ramathuba for ensuring the hospital had access to critical resources, including specialised surgical instruments, medication, and sutures.
“You made it possible by navigating the financial processes and ensuring the team had what they needed,” he said.
Early Detection and Emergency Referral
The twins’ condition was first identified during an ultrasound at Maphutha Malatjie Hospital, prompting the transfer of the 29-year-old mother to Mankweng Hospital for specialised care.
This highlights the importance of early diagnosis and coordinated referral systems in managing complex medical cases.
