The patient, Noorjahan, had initially visited the hospital in Bhojipura for childbirth, which ended tragically with a stillborn. Following this, she experienced severe pain and bleeding, prompting her husband, Tahir, to seek further medical attention.

An ultrasound at a different facility uncovered the presence of the forgotten gauze, leading to an emergency removal and preventive hysterectomy. On acknowledging a formal complaint, the chief medical officer mobilized a probe team and ensured stringent consequences for any verified culpability.
