An investigation into the Chhattisgarh train collision has found that an inadequately certified loco pilot was at fault. The probe report claims the rail administration negligently deployed a loco pilot who failed mandatory tests and sought basic operational guidance during the journey.

The crash involved a MEMU train colliding with a goods train, resulting in 12 fatalities, including the loco pilot, and injuring 19 passengers. Evidence revealed uncontrolled dependence on telephonic instructions due to inadequate competency and certification.

The report calls for reforms in certification processes for loco pilots to ensure this tragedy is not repeated, recommending an Advanced Auxiliary Warning System to enhance train safety. This system could prevent such accidents by automatically managing braking in emergencies.
